What Is a Genogram?

A genogram is a structured diagram that maps family relationships across multiple generations. Unlike a conventional family tree, which focuses primarily on biological descent and names, a genogram captures a richer picture: medical history, emotional relationships, recurring behavioural patterns, and significant life events are all documented within the same visual framework.

The term was coined in the 1980s by family therapist Monica McGoldrick and physician Randy Gerson in their landmark book Genograms in Family Assessment. Their work established a standardised symbol set and a systematic methodology that has since become the foundation of genogram practice in clinical, social work, and educational settings worldwide.

How a Genogram Differs from a Family Tree

A family tree answers the question: who is related to whom? A genogram answers a much broader set of questions:

  • Which family members suffered from hereditary illnesses?
  • Where do patterns of divorce, addiction, or estrangement repeat across generations?
  • What were the emotional dynamics between specific individuals?
  • Which events — migrations, deaths, births — coincide with behavioural shifts in the family system?

This depth of information makes the genogram an indispensable clinical tool rather than simply a genealogical record.

Core Components of a Genogram

Every genogram is built from three fundamental layers:

  1. Structure — the biological and legal relationships between family members, displayed across at least three generations.
  2. Information — names, dates of birth and death, occupations, medical diagnoses, and other personal data attached to each individual.
  3. Relationships — the quality of connections between individuals, drawn with specific line types: close, conflictual, estranged, fused, and so on.

Who Uses Genograms?

Genograms are used across a wide range of professional disciplines:

  • Family therapists and psychologists use them to explore intergenerational patterns and plan therapeutic interventions.
  • Social workers create genograms during intake assessments to understand a client's support network and risk factors.
  • Physicians and nurses use medical genograms to track hereditary conditions such as diabetes, cancer, or cardiovascular disease.
  • Educators and school counsellors use simplified genograms to understand a child's home environment.
  • Researchers employ genograms as a qualitative data-collection tool in family studies.

Why the Visual Format Matters

Information that would fill many pages of case notes becomes immediately comprehensible when rendered as a genogram. Patterns that are invisible in a written report — a recurring pattern of early parental loss, a cluster of cardiovascular disease on one side of the family, a chain of conflictual mother–daughter relationships — jump out visually within seconds.
